    If you own a business and wish to retail our products, please download, fillout, and then fax back to us our application:

  • Dealer Application (Word .DOC file)

    • If you wish to order product from us through FarrowMedical.com, then register with our site and send us an email, Sales(at)FarrowMedical.com identifying your username with your name and/or business.

    Once your application has been accepted, we will then grant you online ordering privileges and you will be notified.

    In the meantime, we highly recommend you view the measuring and donning videos on our website, which will not only aid you in understanding how to measure for and fit our garments, but how our paper order forms work. You can always call with questions.
